Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components.

Job Description :

Essential Functions :

• Develops, implements, and enhances special projects promoting inpatient / outpatient continuity of care and parent engagement.
• Ensures the provision of high quality, individualized, comprehensive, cost effective patient care across the continuum.
• Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives.
• Coordinates functions with interdisciplinary medical care, liaison functions with patient, family, medical staff, and referring physicians.
• Develops educational materials and programs for patients, nursing and ancillary staff, and physicians.
• Develops clinical protocols, policies, and procedures including community education and public relations.

Licensure Requirement :

Registered nurse in the State of Ohio, required.

Experience :

Three years of registered nurse experience, required.
