Job Description American Traveler is hiring a Registered Nurse for a Hospice assignment focused on case management and skilled nursing care for home health hospice patients, requiring at least 2 years of acute care experience.
Responsibilities Work in a home health hospice setting with a patient caseload of 12 to 15 families
Day shift schedule, Monday through Friday, 5x8 hour shifts
Population includes patients and families admitted to hospice care
Position involves case management and comprehensive assessments for hospice admissions, skilled nurse visits, recertifications, and discharges
Use of Epic and Remote Client EMR platforms for documentation
May float to other units as needed
Responsible for providing skilled nursing care and education to patients and caregivers according to the hospice plan of treatment
Coordinates and communicates closely with the interdisciplinary team to ensure smooth flow of care across the continuum
Responsible for wound care management, symptom management, development and management of individualized care plans, and discharge planning
Assists with family bereavement services following patient death
Uses Epic and Remote Client for clinical documentation
Holiday coverage may be required depending on unit needs
Time off requests must be presented for approval prior to signing the contract
Local candidates within 60 miles are not accepted
Requirements Active RN license in Oregon required
Proof of application for Washington state RN license required at the time of consideration
Minimum 2 years recent acute care nursing experience required
Prior hospice experience required for first-time travelers
Fully vaccinated against COVID-19
Ability to provide two professional references from the past two years, with at least one from a supervisory role
No more than one DUI on record
Must have been separated from the previous health system for at least one year if returning as a traveler
