Key Responsibilities

Conduct patient outreach through home visits and establish supportive relationships

Provide health education and assist patients in navigating healthcare resources

Collaborate with care teams and community organizations to coordinate patient care

Required Qualifications

High School Diploma or G.E.D. with 2 years of relevant experience

Ability to work as a team member and engage with diverse populations

Competent in basic computer skills and database entry

Valid driver's license and access to a vehicle

Spanish-speaking skills are a plus
