Are you a compassionate and dedicated social worker looking to make a real difference in the lives of patients and families facing end-of-life challenges? At Mountain Valley Hospice, we provide exceptional care and support, and we’re seeking a PRN Hospice Social Worker to join our team. This role is more than a job—it’s an opportunity to bring comfort, guidance, and advocacy to those who need it most.

What You’ll Do :

• Support & Advocate – Assess patients and families, develop personalized care plans, and provide emotional and practical support to enhance coping skills.
• Educate & Guide – Help families navigate the dying process, anticipatory grief, and bereavement while ensuring they have the resources they need.
• Collaborate & Communicate – Work closely with an interdisciplinary team, including nurses, chaplains, and volunteers, to advocate for patient needs.
• Connect to Resources – Make referrals to community organizations, chaplains, and volunteer programs to provide additional support.
• Stay Organized – Maintain timely and accurate documentation to ensure seamless care for each patient.
