Pennsylvania and New Jersey Licensed Health Coach RN

A company is looking for a Health Coach RN - remote (PA / NJ / DE).

Key Responsibilities

Provide health coaching and case management services to members across the continuum of health

Develop and implement care plans while supporting members in self-management and behavior change

Facilitate communication between patients, families, and healthcare teams, ensuring high-quality service delivery

Required Qualifications

Current, active, and unrestricted Registered Nurse licensure in New Jersey and Pennsylvania

Three years of experience in clinical, case management, and / or disease management

Ability to work flexible hours

Basic computer skills

Strong clinical knowledge
