Required Certifications

BLS, PRI (Patient Review Instrument) Certification, SCREEN Training Certification

Role : Expected to have knowledge of discharge planning and making referrals. Will typically be working on a MS or Tele unit and will split the floor with a Social Worker.

Must be able to function independently on the unit and working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians / team. Need to have a positive attitude, be able to critically think, and good time management skills. Utilization Review (UR) is a separate dept- UR will do insurance approvals and arranging transportation
