Job Title : Physical Therapist (PT)

Job Summary :
The Physical Therapist (PT) is responsible for delivering high-quality physical therapy services to patients, caregivers, and partner organizations. With minimal supervision, the PT evaluates patients, develops treatment plans, provides direct therapy, supervises support staff, and ensures all documentation meets required standards. The PT plays a key role in delivering patient-centered care while maintaining positive relationships with care teams and promoting professional collaboration.
Job Description :
Essential Functions :
Practice in accordance with applicable state laws and regulations governing physical therapy
Evaluate, treat, and direct care for patients requiring medically necessary physical therapy services
Develop individualized treatment plans and adjust them as needed to achieve functional goals
Maintain thorough and timely documentation of treatment goals, progress, and medical necessity
Complete and submit required documentation, including pre-authorizations, progress notes, and billing
Communicate clearly and professionally with patients, families, physicians, and care teams
Maintain compliance with OSHA, Medicare, and other applicable regulatory and professional standards
Participate in team meetings, in-services, and staff development activities as directed
Stay current on professional knowledge, licensure requirements, and continuing education
Maintain current CPR certification in accordance with state licensure regulations
Additional Responsibilities :
Utilize time efficiently to meet productivity standards
Assist with clinical in-services and mentorship when required
Contribute to center operations or special projects as assigned
Support administrative and operational duties in collaboration with the center leadership
Requirements : Skills & Abilities :
Strong leadership and team collaboration skills
Ability to supervise and mentor PT Assistants and Aides
Excellent organizational, time management, and planning capabilities
Effective clinical judgment and problem-solving skills
Clear and professional communication, both verbal and written
Proficient in computer systems for documentation, billing, and scheduling
Physical Requirements :
Ability to lift up to 50 pounds safely using proper body mechanics
Ability to walk, stand, bend, and reach continuously during the workday
Visual and auditory ability to evaluate and monitor patients effectively
Fine motor skills for documentation and equipment handling
Ability to push and maneuver wheeled equipment in clinical and community settings
Credentials :
Graduation from a CAPTE-accredited Physical Therapy program (or equivalent for internationally educated therapists)
Current and active state licensure / registration as a Physical Therapist
Current CPR certification meeting state licensure requirements
