Title :

Registered Nurse (RN)

Location : Kokomo, IN

Summary :

Join a brand-new PACE (Program of All-Inclusive Care for the Elderly) center in Kokomo, IN, dedicated to empowering seniors to age safely and independently in their homes and communities. As an RN, you will be a vital part of the interdisciplinary care team, collaborating with the Medical Director and other team members to deliver holistic, coordinated care tailored to the unique needs of elderly participants. This role offers a rewarding opportunity to help build processes and protocols from the ground up as the center grows its participant census.

Day to Day :

Collaborate with the Medical Director and IDT team to develop and implement individualized care plans

Conduct initial, semi-annual, and annual comprehensive assessments (head-to-toe, hearing, wound / skin exams)

Administer medications and treatments as directed by PCPs

Provide health education and counseling to participants

Perform wound care, IV hydration, blood pressure monitoring, diagnostic testing, and immunizations

Communicate with external referrals and respond to inquiries regarding participants

Participate in daily morning huddles with the interdisciplinary team

Document all care and maintain accurate medical records using IntusCare EHR

Travel as needed within the community for participant assessments

Qualifications :

Graduate from an NLN-accredited nursing program; BSN preferred

Current and valid RN licensure

Minimum 1 year clinical experience in home health, acute care, medical-surgical, nursing home, or similar setting

Minimum 1 year experience working with frail or elderly populations

Strong knowledge of Medicare / Medicaid compliance and value-based care models

Basic Life Support (BLS) certification required

Excellent communication and customer service skills

Monday to Friday, 8 : 00 AM - 4 : 30 PM
