About the Role : We are looking for a dedicated Licensed Mental Health Professional who is passionate about delivering evidence-based clinical services. In this role, you will collaborate with interdisciplinary teams and maintain ethical standards while providing high-quality care across the continuum of services.
Key Responsibilities :
- Conduct comprehensive bio-psychosocial assessments and develop individualized care plans.
 - Collaborate with interdisciplinary teams to ensure appropriate levels of care, develop discharge plans, and facilitate referrals.
 - Utilize therapeutic methods and interventions to meet the unique needs of patients and families.
 - Monitor the progress of therapeutic interventions and adjust care plans as necessary.
 - Educate patients and families about disease prevention, health maintenance, and adherence to treatment goals.
 - Provide direct patient care, including crisis intervention, case management, and emotional support for patients and their families.
 - Maintain accurate and timely documentation that meets regulatory and licensing standards.
 - Engage in administrative and development activities, including clinical supervision and staff education.
 - Support the development of programs to address patients' clinical and strategic needs.
 - Collaborate as a team member to enhance patient experience and outcomes.
 - Ensure cost-effective utilization of resources within your practice.
 
Qualifications :
Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or a related field.Previous experience in a healthcare setting.Current LCSW, LPC, or LMFT license.Valid Driver's License.Proficient in computer systems.Ability to assess issues effectively and develop timely solutions.Strong communication skills with the ability to foster positive relationships.Demonstrated competencies in addressing age-specific patient needs.Commitment to personal safety, patient safety, and ethical practices.Awareness of health-related social needs and cultural competence.Understanding of diverse cultural backgrounds and their impact on healthcare decision-making.Hartford HealthCare offers an extensive benefits package including :
