About the Role : We are looking for a dedicated Licensed Mental Health Professional who is passionate about delivering evidence-based clinical services. In this role, you will collaborate with interdisciplinary teams and maintain ethical standards while providing high-quality care across the continuum of services.

Key Responsibilities :

  • Conduct comprehensive bio-psychosocial assessments and develop individualized care plans.
  • Collaborate with interdisciplinary teams to ensure appropriate levels of care, develop discharge plans, and facilitate referrals.
  • Utilize therapeutic methods and interventions to meet the unique needs of patients and families.
  • Monitor the progress of therapeutic interventions and adjust care plans as necessary.
  • Educate patients and families about disease prevention, health maintenance, and adherence to treatment goals.
  • Provide direct patient care, including crisis intervention, case management, and emotional support for patients and their families.
  • Maintain accurate and timely documentation that meets regulatory and licensing standards.
  • Engage in administrative and development activities, including clinical supervision and staff education.
  • Support the development of programs to address patients' clinical and strategic needs.
  • Collaborate as a team member to enhance patient experience and outcomes.
  • Ensure cost-effective utilization of resources within your practice.

Qualifications :

  • Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or a related field.
  • Previous experience in a healthcare setting.
  • Current LCSW, LPC, or LMFT license.
  • Valid Driver's License.
  • Proficient in computer systems.
  • Ability to assess issues effectively and develop timely solutions.
  • Strong communication skills with the ability to foster positive relationships.
  • Demonstrated competencies in addressing age-specific patient needs.
  • Commitment to personal safety, patient safety, and ethical practices.
  • Awareness of health-related social needs and cultural competence.
  • Understanding of diverse cultural backgrounds and their impact on healthcare decision-making.
