Job Summary : The RN MDS Coordinator conducts and coordinates the assessment process known as the Minimum Data Set (MDS) in long-term care settings. This role ensures that patient assessments are accurate and compliant with regulatory requirements, ultimately affecting care planning and reimbursement.

Key Responsibilities :

• Conduct comprehensive patient assessments following MDS guidelines.
• Compile and analyze data to ensure accurate care planning and documentation.
• Collaborate with the interdisciplinary team to develop individualized care plans.
• Maintain compliance with federal and state regulations related to MDS reporting.
• Educate staff on MDS processes and documentation standards.
• Identify areas for clinical improvement and recommend appropriate interventions.
