In Office " Outpatient position

Monday through Friday 8 : 00am " 5 : 00pm

No weekends

Low patient panel

Comfortable with use of Electronic Medical Records (EcW)

Detect and manage chronic diseases

Develop patient strategy to reduce hospitalization and ER visits

Collaborate with Case Managers and Specialty Providers

Educate patients on the value of compliance

BC / BE Family Medicine Physician

Active Florida medical license

2 years Primary Care experience
