Responsibilities

Fort Lauderdale Behavioral Health Center, a subsidiary of UHS , is a beautiful, conveniently located, private 182-bed hospital that specializes in the treatment of drug addiction, mental health issues and concurrent disorders for adults as well as adolescents. CASE MANAGER (BSW) - Full Time

The Case Manager functions under the direct supervision of the Director of Social Services. He or she assumes responsibility for working with the treatment team and the patient in order to assist in basic discharge planning needs, coordination of aftercare and placement of the patient. This includes residential needs, aftercare needs, advocacy needs, entitlement needs, and building relationship with aftercare providers. Other duties as assigned by supervisor.

Qualifications

Education / Training :

Experience :

A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting, with direct experience in case management patient assessment, patient aftercare coordination, and communication with external review organizations. Experience in working with community resources, residential / outpatient programs and outside referrals.

Knowledge and Skills :

Extensive knowledge of community resources and FLBHC’s referral sources in order to make appropriate referrals.
