RN / Nursing Supervisor

The Nursing Supervisor works under the direction of the Director of Clinical Services / Clinical Manager and provides nursing care to patients as well as provides oversight to the nursing, home health aide, medical social work, physical therapy, speech-language pathology and occupational therapy staff and contractors provided to patients in the home health program.

This oversight must include the following :

• admitting patients, performance supervisory visits, completing recertifications, making patient and personnel assignments; coordinating patient care; coordinating referrals; assuring that patient needs are continually assessed; and assuring the development, implementation, and updates of the individualized plan of care.
• Sets up the plan of care, ensures medical records are maintained appropriately; audits the clinical records and verifies that medical supplies are available in the patient’s home.
• Verifies that care is provided in accordance with licensure, state law, and Medicare / Medicaid standards for Home Health.
• Receives consultation, direction and supervision from the Administrator.
