Job description

Recovery+ Expansion Lpn

Under the supervision of the Medical Director, Advanced Practice Nurse Practitioners, the administrative supervision of the Clinical and Administrative Managers, and in collaboration with the Recovery+ RN, the RECOVERY+ Expansion LPN is responsible for providing an array of nursing care and services for Pathways offices and clients participating in medication-assisted treatment and recovery. These responsibilities include but are not limited to regular travel to outlying Pathways offices, coordinating lab work, injections, screening, and ordering of medication for clients at multiple Pathways offices, providing ongoing client education and support, and documenting care in the client's record. Specifically, the Recovery Expansion Peer is an integral player in ensuring the smooth roll-out and operation of the RECOVERY+ Expansion project into Pathways' surrounding county clinics. The Recovery+ Expansion LPN maintains a weekly schedule equivalent to thirty-six (36) to forty (40) hours per week. Incumbents are subject to regular travel to other Pathways offices and locations in West Tennessee.

Essential Job Functions :

  • Provides client and family education, support, and care in gender-, spiritually-, economically-, and culturally-responsive, humble, kind, and competent ways
  • Follows HIPAA guidelines and maintains the highest level of confidentiality and privacy at all times when dealing with client information
  • Coordinates daily with the RECOVERY+ home office while expanding services onsite at outlying Pathways outpatient locations to deliver services and close loops in conjunction with the R+ home office team
  • Assists in managing and maintaining provider schedules and client appointments
  • Facilitates equitable services for clients in person and via telehealth as needed
  • Conducts thorough and competent screening for licensed physicians and nurse practitioners via telehealth, telephone, and face-to-face interview
  • Administers injections and secures specimens for laboratory tests as ordered by a licensed physician or nurse practitioner
  • Appropriately documents all activities as well as physician and nurse practitioner orders promptly with correct and appropriate tone, grammar, and syntax
  • Assists in the coordination, monitoring, and documentation of the ordering, shipping, logging, administration, and usage of medication as ordered by a licensed physician or nurse practitioner
  • Completes Prior Authorizations as needed or directed by a licensed physician or nurse practitioner
  • Communicates effectively and appropriately with providers, pharmacies, managed care companies, patient assistance programs, pharmaceutical companies, and partnering organizations and entities, documenting as needed
  • Responds to medical emergencies, initiates CPR or appropriate action, and notifies authorized personnel
  • Collaborates effectively with service recipients, outside agency personnel, family members, and internal agency personnel including but not limited to supervisors, therapists, physicians, nurses, and case managers to ensure continuity of care
  • Provides reporting of work activity to Clinical Manager as requested
  • Participates in staffing and supervision as scheduled, and displays evidence of the value of lifelong learning as related to best practices in client care
  • Participates in Quality Assurance & Improvement, Utilization Management, and other agency and system functions
  • Other activities or responsibilities as needed per supervisor or agency including but not limited to cross-training with and offering services for other departments

Job Specifications :

  • Education : Skills and proficiency in the technical field of practical nursing (such proficiency is acquired through completion of a 1-year training program in a vocational school LPN program)
  • Licensure, Registration, Certification : Current licensure as Licensed Practical Nurse by the State of Tennessee; BLS certification must be obtained within the orientation period, and certification must be kept current while in this position; Must have a valid driver's license and the ability to be granted driving privileges of company vehicles by West Tennessee Healthcare; Any additional certification or training as requested by supervisor or Agency
  • Experience : Proficiency in pharmacology and charting and ability to demonstrate nursing skills according to current hospital and nursing procedures and policies as acquired through successful completion of charting class, pharmacology exam, and skills exam
  • Knowledge, Skills and Abilities :

  • Exhibits proficiency in Microsoft Word, Excel, Outlook, and PowerPoint
  • Exhibits capacity to learn and utilize electronic medical records and other online software programs quickly and effectively
  • Demonstrates professional written, verbal, and nonverbal communication
  • Demonstrates ability to set priorities for tasks, to work effectively under minimal supervision, exceptionally organized, described by others as a self-starter, reliable, dependable, and motivated
  • Demonstrates eagerness and ease in learning and implementing new technologies, strategies, and interventions
  • Demonstrates an understanding of adverse childhood experiences (ACEs), their potential effect on the health and well-being of individuals, and utilizes best practices to offer trauma-sensitive and trauma-informed care
  • Interpersonal Requirements :

  • Demonstrates skills in partnership & problem-solving : identifying site-specific issues (space, scheduling, pharmacy coordination) and collaborating on fixes with the outlying office and the home office.
  • Demonstrates the ability to communicate effectively and build rapport with clients and family members, as well as all levels and facets of agency personnel
  • Contributes to a positive work environment and the overall team effort of Recovery Programs, Pathways, and West Tennessee Healthcare
  • Flexible and innovative, exhibiting the initiative to function with limited supervision
  • Addresses interpersonal conflict concerning colleagues or supervisors with humility, respect, and frankness, following the chain of command when needed to resolve issues effectively and timely
  • Identifies difficulty in client tone, words, behaviors, expectations, requests, and demeanor in the context of client substance use disorder and responds therapeutically and professionally at all times

  • Physical Demands :

  • Able to perform various physical demands of the job that help deliver services to program recipients and the agency. Physical demands include but are not limited to prolonged standing, sitting, walking, driving, climbing stairs, lifting / carrying objects, etc.
  • Able to use crisis de-escalation training to effectively de-escalate and safely restrain an individual if needed
  • Able to work hours as designated by client need and provider availability when needed
  • Environmental Conditions :

  • Possible exposure to contagious disease
  • Continuous exposure to human waste, specimens, blood, wounds, soiled linen, and needles
  • Prepared to travel throughout the agency's catchment area or where consumers reside to provide services including but not limited to locations such as other offices, criminal justice complexes, etc. on a regular basis
