We are currently seeking a RN Case Manager in the Anaheim area who shares our passion for providing dignified, compassionate end-of-life care. This person will be covering the North / Central Orange County areas.
Shift Schedule : Full Time, Monday-Friday, 8 : 00 AM - 5 : 00 PM

Qualifications :
Must be a Registered Nurse with a valid state license
One year of experience working with your RN license required
Hospice experience preferred, but not required
This role requires a valid driver’s license and a good driving record as a condition of employment. Associates must comply with all state and federal driving regulations
Associates in roles requiring driving are responsible for maintaining a valid and active driver’s license. Failure to maintain a valid license may result in reassignment, suspension, or termination in accordance with company policies and applicable laws
Reliable transportation and willingness to drive to patient’s location
Must be able to lift 50 pounds
Must clear a criminal background check, physical and drug screening
You may be required to work outside of your regular hours depending on business needs
What you'll be doing :
Develop a detailed Plan of Care for assigned patients ensuring all services needed are identified and met while working closely with the Interdisciplinary Group, patient, family and attending physician
Responsible for the coordination of care for the patient and family, providing skilled Hospice and Palliative care for the remainder of patient’s life
Provide comprehensive nursing evaluations and assessments
Provide field supervision to LVN’s and Home Health Aides assigned to your patients
Filed case management
Other duties as assigned
Anticipated pay range : $55 USD - $58 USD
