Description :

ALL APPLICANTS MUST HAVE 1 YEAR CERTIFIED HOME CARE EXPERIENCE.

1 YEAR CASE MANAGEMENT EXPERIENCE IS REQUIRED.

Ability to travel to cases as assigned is necessary.

Weekend rotations occur once every 6 weeks.

As a Home Health RN, responsibilities include assessing patient conditions.

Responsibilities also involve implementing care plans.

Providing ongoing education and support to patients and their families is required.

Patient care is delivered within organizational philosophy, policy, and standards of community health practice.

The role includes performing admission processes.

Case management responsibilities must align with established policies and standards of practice.

Thorough and timely electronic documentation is essential.

Interdisciplinary communications are an integral part of the role.
