• Starting ASAP September, 2018
• Location : Nearest airport is Chicago Rockford Airport (30 minutes from hospital)
• Hours : Monday- Friday, 8a-5p
• Hospital : 103 beds, joint commission accredited, non-profit
• Support services : Counseling, occupational therapy, physical therapy, palliative care, ostomy, prosthetics, and integrative therapies
• Equipment : Elekta Infinity with agility head, elekta response gating interface for C-Rad or ABC, Mosaiq, pinnacle treatment planning system, 3 camera catalyst HD system in treatment room, Mobius, MIM, Active breathing coordinator, Hexapod 6 degree couch, Siemens somatom definition AS CT, Sentinel camera from C-Rad in CT room for 4D motion management, Anzai belt that came with CT scanner, Delta 4, Sun nuclear 3D water tank / scanner, IC profiler, Body fix system
• Required : Board certification or Board eligible, Wisconsin license
