As the HCV Patient Navigation Coordinator , you will play a pivotal role in individuals' lives, making a significant impact through :

  • Review completion of daily temperature checks
  • Review completion of internal controls according to state and agency guidelines
  • Provide Pre and Post-test counseling
  • Perform HCV screening and testing in accordance with state, agency guidelines, and contract guidelines
  • Perform and support Project Safe Points Point of Care testing through the GeneXpert machines

HCV Patient Navigation Coordinator Minimum Salary : $ 58,500

HCV Patient Navigation Coordinator Maximum Salary : $ 58,500

HCV Patient Navigation Coordinator Schedule : Monday - Friday 8 : 30 AM - 4 : 30 PM (37.5 hours per week)

Qualifications :

  • Bachelor's Degree in Health Education or Human Services
  • 1-3 years of relevant experience in Harm Reduction and related fields
  • Valid NYS Driver's License, including a reliable vehicle
