The Licensed Practical Nurse (LPN) responsibilities include-

• Maintain and document clients medical necessity.
• Admit and Discharge clients.
• Complete Admission documentation for clients enrollment.
• Manage clients Med-list upon admission through discharge.
• Work with Clinical team on clients treatment goals.
• Conduct education groups with proper documentation as needed.
• Monitor and document the effectiveness of treatment and medication.
• Maintain a courteous, and professional appearance at all times.
• Follow facility policy and procedures as outlined in the Employee Handbook.
• Follow a comprehensive plan of care for individual clients.
• Coordinate with Home Health facilities.
• Other duties may apply.
• To work from 6am-2pm and 4pm -10pm
