LPN - Long Term Care (LTC)

Job Description :

Position Summary : As a Licensed Practical Nurse (LPN) in our long-term care facility, you will play a vital role in ensuring the well-being and comfort of our residents. You will be responsible for providing direct nursing care, administering medications, and collaborating with the interdisciplinary healthcare team to develop and implement individualized care plans.

Key Responsibilities :

• Patient Care :

Provide high-quality nursing care to residents with various medical conditions and needs.

Maintain accurate and up-to-date medical records, including residents' charts and care plans.

Collaborate with registered nurses (RNs), certified nursing assistants (CNAs), and other healthcare professionals to develop and revise care plans.

Adhere to all applicable healthcare regulations, including HIP nd state nursing guidelines.
