Primary Care Nurse Practitioner Location : Chesapeake, Virginia Practice : Chesapeake Internists Limited

Position Overview :

Chesapeake Internists Limited, a well-established internal medicine practice in Chesapeake, Virginia, is seeking a dedicated Primary Care Nurse Practitioner (FNP or AGNP) to join our team. This full-time position operates Monday through Friday, 8 : 00 AM to 5 : 00 PM, providing comprehensive primary care services to adult patients aged 18 and older.

Key Responsibilities :

• Conduct comprehensive assessments and develop individualized care plans for adult patients.
• Manage common acute and chronic conditions, including hypertension, diabetes, and respiratory illnesses.
• Perform routine physical examinations and preventive health screenings.
• Provide patient education on health maintenance, disease prevention, and lifestyle modifications.
• Conduct women's health screenings, including Pap smears and pelvic exams.
• Collaborate with physicians and other healthcare professionals to ensure coordinated care.
• Utilize electronic health records (EHR) for accurate and timely documentation.
