Clinical Resource Coordinator

RN Care Managers are responsible for delivering clinically-based case management services that support the efficient delivery of patient care under the direction of a senior leader. They have accountability for integrated responsibilities including utilization review, discharge planning, coordination of care, and ensuring high-quality patient outcomes.

Key responsibilities include identifying appropriate resource utilization, confirming medical necessity for admission and continued stay, and creating a discharge plan in conjunction with the patient, family, and healthcare team.

The successful candidate will collaborate with other members of the healthcare team to ensure high-quality patient care, utilizing established criteria and protocols to inform decision-making.

Required Skills :

• Nursing degree from an accredited educational institution
• Current Washington State Registered Nurse License
