Job Summary

The Geriatric Care Manager is responsible for assessing, planning, coordinating, and monitoring services for older adults and their families. This role combines client-centered care planning, quality assurance, advocacy, problem-solving, and coordination with caregivers, family members, and community partners to promote client well-being and independence. The role focuses on enhancing the quality of life for seniors by providing personalized care management, advocacy, and support while helping families navigate aging-related challenges.

Position Responsibilities

  • Conduct comprehensive assessments of clients’ physical, emotional, social, and environmental needs, including in-home safety planning.
  • Develop and implement individualized care plans tailored to client goals and needs.
  • Coordinate and monitor services such as home care, medical appointments, housing arrangements, and legal / financial resources.
  • Provide ongoing case management, crisis intervention, and advocacy for clients.
  • Serve as a liaison between clients, families, healthcare providers, and community resources.
  • Educate families on aging-related issues, care options, and available support services.
  • Respond to emergencies and changes in client conditions with appropriate action.
  • Facilitate support groups, psychoeducational workshops, and training.
  • Maintain accurate, confidential client records and documentation.
  • Conduct chart reviews and auditing as required.
  • Participate in interdisciplinary team meetings and community outreach.
  • Attend mandatory agency-wide meetings and training.
  • Required Minimum Qualifications

  • Master’s degree in social work, counseling, rehabilitation, gerontology, or related field.
  • 3+ years of professional experience working with geriatric clients and their families.
  • Demonstrated knowledge and skills in geriatric care management and home care social work.
  • Strong understanding of aging issues, dementia care, chronic illness management, and available community resources.
  • Committed to culturally informed practices and a continuous learning environment
  • Must have reliable transportation. When using a personal vehicle for work-related duties, you are required to provide proof of a valid driver's license, current vehicle registration, and automobile insurance coverage as required by California law.
  • Preferred Qualifications

  • Licensed Clinical Social Worker (LCSW), RN, or other relevant licensure.
  • Certification as a Care Manager (CMC) through the Aging Life Care Association (ALCA) or National Academy of Certified Care Managers (NACCM).
  • Experience with Medicare, Medicaid, and long-term care systems.
  • Knowledge of Jewish culture, traditions, and community resources.
  • Experience facilitating multidisciplinary team collaboration.
  • Experience using outcome-based measurement tools and quality assurance practices in social services or healthcare.
  • Position Funding & Stability

    This position is a limited term grant funded position with an anticipated end date of June 30, 2026, but may be eligible for an extension based on funding. This position is funded through a long-standing contract that has historically been renewed. While we remain confident in our continued partnerships, we believe in full transparency with prospective team members. The broader landscape of community-based services is currently shifting, with potential changes at state and federal levels that may impact funding structures in the future and our ability to extend this position.

    Compensation and Benefits

    The compensation for this position will range from $41 - $46 / hour based on experience.
