Responsibilities :
The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.
Responsibilities :
- Assessment and Evaluation : Conduct comprehensive physical assessments to determine residents' physical impairments and functional limitations, focusing on mobility, balance, and gait.
- Individualized Care Plans : Develop individualized treatment plans based on assessments, with clear goals for improving residents' functional mobility.
- Section GG Data Collection : Accurately and consistently collect Section GG data as required for Annual, Quarterly, Falls, and Significant Change assessments.
- Treatment : Administer physical therapy interventions, including exercises, manual techniques, and modalities, to enhance residents' physical function.
- Education : Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
- Documentation : Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO / Manage care progress updates in EMRs( InnovaHealth / Net Health / PCC).
- Interdisciplinary Collaboration : Collaborate with the rehab team, including Occupational Therapists and Speech-Language Pathologists, to ensure comprehensive care for residents.
Qualifications :
State licensure as a Physical Therapist.EMR and MS office applications.CPR / BLS certification.Knowledge of Section GG data collection and documentation requirements.Strong clinical and interpersonal skills.Excellent written and verbal communication skills.Ability to supervise assistants, technicians / aids and student / interns.Modality Certification +Experience in LTPAC / LTC clinical programs are always a plusApply today!
