Job Title : Part-Time Physical Therapist (Home Health)

Locations : Tarrant, Denton, Dallas, Collin counties

Position Overview : We are seeking a skilled and compassionate Physical Therapist to join our team on a part-time basis. As a Home Health Physical Therapist, you will play a vital role in improving the quality of life for our patients by providing specialized rehabilitative care in the comfort of their homes. This position offers the opportunity to work with a diverse patient population, collaborate with interdisciplinary teams, and make a meaningful impact in the community.

Responsibilities :

• Conduct comprehensive evaluations to assess patients' physical abilities and limitations.
• Develop individualized treatment plans tailored to meet patients' specific needs and goals.
• Provide skilled physical therapy services, including therapeutic exercises, manual therapy techniques, gait training, and functional mobility training.
• Educate patients and their caregivers on therapeutic exercises, safety measures, and techniques for maximizing independence.
• Regularly reassess patients' progress and adjust treatment plans as necessary.
• Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment outcomes.
• Collaborate effectively with physicians, nurses, and other healthcare professionals to ensure coordinated and integrated care.
• Adhere to all regulatory requirements and ethical standards governing physical therapy practice.
