JOB SUMMARY

As a Nurse Practitioner (NP) or Physician Assistant (PA) at Catalyst Physician Group, you'll be a vital part of our team, helping our communities thrive by providing direct patient care and supporting clinical operations. We're a purpose-driven organization, passionate about making a positive impact on the lives of our patients and providers. You will see approx. 15-25 patients per day and have in office support as well as Catalyst support teams including 24 / 7 care team support, referral coordinators and more.

ROLE AND RESPONSIBILITIES :

Functions independently to perform a broad range of medical services including, but not limited to the following :

• Providing direction, coordination, and evaluation of patient care delivery
• Patient assessment
• Age appropriate history and physical
• Diagnosing and treating acute and chronic conditions
• Ordering and interpreting diagnostic tests such as lab work and x-rays relative to a patients specific needs
• Annual exams
• Educating patients on disease prevention and positive health and lifestyle choices
• Documenting encounters in the Athena EMR
