Connections to Care Case Manager – Substance Abuse Disorder Case Manager

Emergency Shelter, Syracuse NY

Shift : Sundays-Thursdays 7 : 30am-4 : 00pm (alternating Wednesdays : 10 : 00am-6 : 30pm)

Rate of Pay : $23.00-25.00 per hour

The Rescue Mission is seeking a Connections to Care Case Manager to work in our Emergency Shelter located in Syracuse, NY . This is an exciting opportunity to make a meaningful impact by supporting individuals in their journey toward recovery and self-sufficiency. Your job is to assist clients by assessing Substance Use Disorder needs and collaborating with provider networks to identify addiction treatment, harm reduction, and recovery services that align with client goals. This role is crucial in maintaining a structured, safe, and supportive environment that reflects the mission, vision, and values of the Rescue Mission.

Duties & Responsibilities :

The Connections to Care Case Manager will manage a caseload of clients, providing essential support and resources to foster their substance use recovery.

Key responsibilities include :

  • Conducting substance use disorder assessments to evaluate client strengths, challenges, and service needs.
  • Providing supportive counseling to guide clients toward personal growth, goal setting, and suitable recovery.
  • Working closely with internal and external service providers to coordinate treatment, harm reduction, and recover services tailored to each client’s needs.
  • Monitoring client progress regularly to ensure goals are met.
  • Building and maintaining strong relationships with community partners in areas such as mental health, addiction services, legal, and financial support.
  • Ensuring confidentiality and professional boundaries in all client interactions.
  • Responding promptly to communications from partner agencies, ensuring collaboration and coordination.
  • Attending required meetings and events.
  • Adhering to safety policies and regular attendance at in-person team collaboration.

Requirements :

  • Bachelor’s degree in social work or related field preferred.
  • Associate’s degree in human services and CASAC training eligibility will also be considered.
  • Valid NYS Driver License in compliance with company driving criteria.
  • Familiarity with community services and networks is a plus.
  • Bi-lingual skills are a plus.
  • Must adhere to CDC Guidance when necessary.
  • Full vaccination, including boosted (when eligible,) against Covid-19 is strongly encouraged.

  • Strong communicate skills, both written and oral.
  • Excellent interviewing and assessment skills to evaluate client needs effectively.
  • Ability to use critical thinking and problem-solving skills.
  • Strong time management skills to handle a dynamic workload effectively.
  • Proficient in Microsoft Office and other relevant computer tools.
  • Ability to work effectively as a team player and collaborate with others
  • Passion for the mission and vision of the Rescue Mission and a commitment to helping others succeed.
  • Rate of Pay : $23.00 - $25.00 per hour
