Coordinator, Post-Acute Network Integration

Position Summary : Under the direct supervision of the Manager, Network Integration (hereafter referred to as Manager), the Coordinator, Post-Acute Network Integration (hereafter referred to as "Coordinator") serves as the primary liaison for the HHC post-acute care tiered network of non-HHC providers (SNF / Home Health / Outpatient Rehabilitation / Other). In addition to supporting transitions for HHC hospital complex care patients, in collaboration with CM teams, this person drives adherence to quality and performance metrics, leads collaborator weekly regional huddles and works collaboratively as a liaison with post-acute providers, Institutes and Care Management teams for the HHC system as related to post-acute care network transition management.

Education Minimum : Associate's degree Preferred : Bachelor's degree in business or related field
Experience Minimum : Experience managing multi-faceted projects-preferred At least 3 years of experience with post-acute care network development and management Preferred : Referral processing / workflow management experience in a medical / physician practice Experience working in an office / administrative leadership capacity in a medical / physician practice
