You will promote the use of evidence-based practice through the nursing process, protocols, and procedures. You will assess the patient's status through interview, observation, and physical assessment of actual problems and outcomes. You will develop an individualized plan of care using a collaborative approach with the patient and healthcare team. You will use appropriate interventions identified in the plan of care to achieve expected patient outcomes. You will systematically evaluate and document the patient's progress toward expected outcomes and goals on an ongoing basis throughout the shift. You will modify the plan of care based on evaluation and changes in patient condition as needed. You will complete required documentation appropriately, including patient chart and event reports. You will demonstrate professional accountability for your practice by seeking resources and continued self-development. You will demonstrate technical knowledge of equipment and troubleshoot when problems arise. You will acquire and maintain current knowledge and skills in evidence-based nursing practice. You will recognize the need for assistance and utilize resources, including consultation, in a timely manner. You will demonstrate ownership and persistence to ensure problems are resolved in the best way possible. You will assume assignments of non-complex and some complex patient management with some turnover in assignments. You will consistently prioritize work based on changes in patient condition, changes in assignments, and unanticipated interruptions. You will collaborate with the healthcare team in organizing tasks and activities for the patient's care from admission to discharge according to the plan of care to meet identified expected outcomes. You will utilize communication and teamwork strategies aimed at promoting high team performance and achieving optimal patient outcomes and unit productivity. You will assess and choose teaching strategies appropriate to the patient's and family's age-specific needs. You will adapt teaching strategies based on the patient's response, readiness to learn, and level of comprehension. You will take cultural, age-specific, and developmental factors into consideration when teaching patients about procedures, illnesses, and the hospital experience. You will ensure patient safety in the hospital environment by following policies and procedures and adhering to National Patient Safety Goals. You will consult appropriate resources within the hospital to provide specialized education to patients and family members. You will actively participate in data collection, analysis, and problem-solving and give input on process improvements and patient satisfaction. You will assure that care provided is consistent with standards of care, evidence-based practices, and patient safety goals. You will work with the interdisciplinary team in identifying opportunities for improvement in patient care and patient / family satisfaction. You will actively promote and achieve customer satisfaction for patients, families, departments, and physicians. You will participate in the implementation of Performance Improvement initiatives. You will promptly and efficiently follow up on patient and family requests and needs, anticipating those needs. You will encourage coworkers in achieving excellent customer satisfaction and utilize organizational service excellence standards. You may serve on unit-based councils and identify practice problems on the unit. You will implement changes in your clinical practice based on literature and unit process improvement outcomes. You may work on assigned projects, data collection, and audits assigned.
