RN Long Term Acute Care LTAC

The Long-Term Acute Care Registered Nurse provides planning and delivery of direct and indirect patient care to medically complex patients in long-term acute settings.\n\nEssential Duties and Responsibilities : \n

• Provides planning and delivery of direct and indirect patient care\n
• Responsible for providing care to medically complex patients\n
• Supervises care, treatment, and services of the patient(s)\n
• Documents the patient's plan of care using identified nursing diagnosis, expected patient outcomes and selected nursing interventions\n
• Maintains confidentiality at all times with patients, medical staff and co-workers\n
• Provides a safe, comfortable and therapeutic patient environment\n
• Adheres to and supports the policies / procedures / goals / objectives of the hospital in an attempt to provide quality patient care\n
• Revises plan of care according to evaluation, changes in plan of care and nursing interventions\n
• Performs interventions according to identified priorities, plan of care, and facility policies and patient care outcome standards\n
• Identifies and prioritizes actions based upon patient care requirements\n\nMinimum Requirements : \n
• Current RN license for the state in which the nurse practices in\n
• One (1) year nursing experience in a long-term acute care facility preferred\n
• Experience in acute rehabilitation for vents, wound care, cardiac, and stable post trauma patients\n
• Complies with all relevant professional standards of practice\n
• Participation and completion of Amergis' Competency program when applicable\n
• Current CPR if applicable\n
• TB questionnaire, PPD or chest x-ray if applicable\n
• Current Health certificate (per contract or state regulation)\n
• Must meet all federal, state and local requirements\n
• Successful completion of new hire training as applicable to job site\n
• Understand patient confidentiality and HIPAA requirements\n
• Ability to effectively elicit / provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required\n
• Computer proficiency required\n
