Wound Care Nurse Practitioner

Job Description

Seeking a qualified Wound Care Nurse Practitioner to treat patients across Southern California in home and long-term care settings.

• Must be a graduate of an accredited NP or PA program with an active California license and board certification.
• Prior wound care experience or certification required; specialty experience in hospice, home health, or assisted living preferred.
• Willing to travel throughout Southern California; excellent communication, documentation, and patient-centered approach essential.
• Perform physical exams, wound assessments, debridement, and prescribe treatments; document findings and follow-up care.
• Must be able to lift 50 lbs, have full mobility, a valid driver’s license, and proficiency with medical equipment.
• Flexible scheduling with travel to patient homes or facilities; compensation per visit.
