Job Summary and Qualifications

As an RN Case Manager, you will play a crucial role in supporting patients and their families as they navigate their healthcare journey. Your responsibilities will include coordinating services, connecting resources, and developing personalized care plans that address individual patient needs. Collaborating with physicians, nurses, and department leaders, you will ensure safe transitions, effective communication, and quality care throughout the process.

Your responsibilities will include:

• Guiding patients and families through program orientation, detailing rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights.
• Coordinating educational support for patients, families, and caregivers, encouraging engagement in treatment planning and goal discussions.
• Creating and implementing customized treatment plans based on patient strengths, needs, and recovery objectives.
• Performing psychosocial assessments and discharge planning evaluations, maintaining clear documentation and communication with the care team.
• Partnering closely with the Rehab Program Director and interdisciplinary team members to ensure cohesive, patient-centered care.
• Upholding HCA Healthcare's core values of compassion, respect, and excellence in each patient interaction.
