About The Position
Position Overview :
We are seeking an experienced Case Manager with an active Massachusetts (MA) license in hand to join our team. This role is essential in supporting hospital discharge planning processes, ensuring smooth care transitions, and enhancing continuity of care for patients. Experience in Long-Term Acute Care (LTAC) or rehabilitation settings is highly preferred.
Key Responsibilities :
Lead and coordinate hospital discharge planning for patients
Collaborate with interdisciplinary teams to ensure timely and effective patient transitions
Monitor and manage patient care plans, ensuring compliance with CMS standards for Continuity of Care / Transition of Care
Document case management activities and patient progress in the Electronic Medical Record (EMR) system, in accordance with ACMA SOP
Advocate for patients while working closely with families and healthcare providers to optimize patient outcomes
Identify barriers to discharge and proactively work to resolve them
Qualifications :
Must have an active Massachusetts (MA) Case Manager license at the time of application
Minimum of 1 year of experience in Case Management in a hospital setting, specifically related to discharge planning
LTAC or Rehabilitation Hospital experience is a plus, but not required
Proficiency in EMR systems and thorough understanding of documentation protocols
Knowledge of CMS standards regarding Continuity of Care and Transition of Care
Preferred Skills :
Strong interpersonal and communication skills
Ability to work independently and collaboratively in a fast-paced environment
Critical thinking and problem-solving abilities
