Transitions of Care Coordinator

A company is looking for a Transitions of Care Coordinator to support members navigating the healthcare system after hospital discharge.

Key Responsibilities

Check in with members post-discharge to ensure they have received necessary assistance

Coordinate with providers and agencies to facilitate member care and resources

Provide operational support to the TOC clinical team and participate in team activities

Required Qualifications

Medical Assistant Certification preferred; medical office experience is highly desirable

Minimum of 2 years in a healthcare setting, preferably in managed care or community health

Fully bilingual in English and Spanish preferred
