Active NE license - HIGHLY PREFERRED Clean Malpractice - HIGHLY PREFERRED Availability of dates in the job description - REQUIRED, NOTE AT NAME CLEAR. Case logs are required - REQUIRED, INCLUDE AT PRESENTATION. Disclose if the provider has worked at any other CHI / Dignity facility - REQUIRED, NOTE AT NAME CLEAR.
