Job Description
We are seeking a compassionate and dedicated Outpatient Dialysis Social Worker to join our multidisciplinary team. In this vital role, you will provide psychosocial support including counseling and case management, education and advocacy for patients with chronic kidney disease receiving dialysis treatment on an outpatient basis or in the home setting. You will collaborate with medical professionals to ensure holistic, patient-centered care and help patients and their families navigate the complexities of chronic illness management. You will complete assessments and surveys per Mayo Clinic Policy / Procedure and CMS requirements and participate in dialysis clinic quality improvement processes.
Qualifications
To be considered for this position, you must have a Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree, and have LCSW licensure to practice independently.
