Are you a compassionate and skilled Registered Nurse (RN) ready to make a difference in your local community? SALMON Home Care is seeking a full-time Home Health RN to deliver personalized care and build meaningful relationships with patients in the Medway / Franklin area.

What You'll Do as a Home Health RN
- Provide skilled nursing care to patients in their homes
- Complete initial assessments and develop individualized care plans
- Administer medications, IV therapy, wound care, and other clinical treatments
- Educate patients and caregivers on managing chronic conditions, medication use, diet, and post-acute care
- Coordinate care with physicians, case managers, and the broader home health team
- Ensure timely and accurate clinical documentation
Hiring Locations :
Full-time hours (32-40 / week) with flexible AM start times to support your lifestyle and scheduling needs. Mileage reimbursement included.
