HUD Service Coordinator – Hartland / Oconomowoc Area

Location : Hartland, WI (with travel to Oconomowoc)

Schedule : Full-Time | 40 hours / week

Employment Type : Onsite | Benefit-Eligible

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate and resourceful HUD Service Coordinator to support residents across three HUD properties in the Hartland / Oconomowoc area . This role is based at Breezewood Commons in Hartland and includes travel to Hickoryview Commons and Parkland Commons in Oconomowoc. ?? (Mileage reimbursed!)

About the Role

As a HUD Service Coordinator, you'll empower residents to maintain independence and self-sufficiency by connecting them with vital community resources. You'll serve as a key liaison between tenants and service providers, helping to reduce early admissions to assisted living or nursing homes.

Key Responsibilities

  • ?? Provide informal case management, intake, and referrals
  • ????? Assess residents' health, psychological, and social needs
  • ?? Develop and monitor individualized service plans
  • ?? Build partnerships with local service providers
  • ?? Educate residents on available services and application processes
  • ????? Coordinate wellness workshops and tenant education sessions
  • ?? Maintain accurate documentation and progress reports
  • ?? Travel between assigned properties

Site Breakdown

  • Breezewood Commons (Hartland, WI) : 20 hrs / week
  • Hickoryview Commons (Oconomowoc, WI) : 10 hrs / week
  • Parkland Commons (Oconomowoc, WI) : 10 hrs / week

    • Qualifications

  • Bachelor's degree in Social Work, Sociology, Psychology, or related field
  • Minimum 3 years of experience working with older adults or individuals with disabilities
  • Strong knowledge of community resources and supportive services
  • Valid driver's license and reliable transportation
  • Proficient in written and verbal communication
  • Comfortable using computers for documentation and communication
