DUTIES OF POSITION
- Provides personal care and related services in the home, under the direction, instruction, and supervision of the Supervising Nurse.
 - Tasks to be performed by an caregiver must be assigned by and performed under the supervision of an RN who will be responsible for the client care / services provided by the caregiver.
 - Under no circumstances may an caregiver be assigned to receive or reduce any intravenous procedures, or any other sterile or invasive procedures.
 
POSITION RESPONSIBILITIES
Follows the plan of care / service to provide, safe, competent care / service to the clientPerforms the following tasks as assigned by the RN :Assist with the activities of daily living
Medication remindersTaking and recording vital signsRecording and reporting to the supervisor any changes regarding the clients condition. behavior or appearanceDocumenting the services delivered in the clients recordHelps the client to maintain good personal hygiene and assists in maintaining a healthful, safe environmentPlans and prepares nutritious mealsAssists the client with ambulation as approvedEncourages the client to become as independent as possible according to the care planAttempts to promote client's mental alertness through involvement in activities of interestGives simple emotional and psychological support to the client and other members of the household and establishes a relationship with client and family which transmit trust and confidentialityCompletes at least 12 hours of in-service training and education annuallyPerforms routine housekeeping tasks as related to a safe and comfortable environment for the clientPrepares a visit report on the day the service / care was providedConfirms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinatedWorks with personnel of other community agencies involved in the client's careConfer with the Supervisor about clients status and care, participate in the planning of servicePromptly report any change in client condition to the SupervisorWorks within the caregivers scope of practice, as directed by a Registered Nurse and report in writing to the Supervising NurseJOB CONDITIONS
The ability to drive and the ability to access clients homes which may not be routinely wheelchair accessible are requiredHearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client careOn occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and / or carry up to 30 poundsMust be able to communicate clearly, both verbally and in writingEQUIPMENT OPERATION
