Discharge Coordinator, 24 Hours

Main Function : Serves as liaison between Case Management Department and Extended Care Facilities. Provides Clerical Support to the Case Management Department

Education : High school education; additional courses in business and / or medical related area preferred.

Experience : Minimum two-year office experience required; excellent interpersonal, telephone and organizational skills required.
