About The Position
Are you looking for a Full time NP position with great benefits?
We are seeking Full time Nurse Practitioners for a primary care opportunity at our Outpatient clinic in Reading, PA. The schedule is Monday-Friday 8a-5p; no nights and no weekends.
What We Offer :
- Competitive Salary (Ranges from $110,000 - $165,000 based on experience and location) with quarterly bonus based on quality metrics
- 6 weeks of PTO, inclusive of vacation, sick time, major holidays, and continuing medical education (CME)

Required Qualifications :
National certification in at least one of the following specialties : Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse PractitionerActive, non-probationary state Nurse Practitioner License1+ year direct primary care experience as a Nurse PractitionerActive DEA licenseRole Summary :
Provision of exceptional primary careConduct office visits for routine and acute issues, and home visits as neededAdminister Annual wellness visits and health risk assessmentsCare coordination with other providers, specialists, testing facilities, and agenciesPopulation health leadership, in coordination with the Care TeamAssisting the care team with phone triage and outreachEducating patients on their health conditions, care plans, and treatmentsIf you are interested in joining our team, apply to this job posting and we will contact you right away!
