Experience Requirements (3+ Years)

  • Direct patient care.
  • Telehealth policies, workflows, protocols, and program development.
  • Quality, outcomes, and data collection.
  • Telehealth technology, remote monitoring, EMR, and telepresence.
  • Identifying patient safety risks and changes in clinical status.
  • Communicating telehealth concepts; understanding language, culture, disability needs.

Education

  • Associate or Bachelor's in a health-related field (preferred).
  • Summary

  • Supports virtual patient care, remote monitoring, and telemedicine services.
  • Collaborates on telehealth program development, training, workflows, and quality / safety initiatives.
  • Communicates with clinical teams, patients, Digital Tech Partners, engineering, and vendors.
  • Key Duties

  • Perform quality assurance, peer reviews, and recommend improvements.
  • Meet monthly with leadership on Virtual Hospital Program metrics and workflow updates.
  • Lead case reviews and monitor outcomes; triage / assign virtual monitoring cases.
  • Develop and implement telehealth policies, workflows, and maintain operational databases.
  • Identify trends and support corrective action plans.
  • Train new staff on telehealth workflows and platform use.
  • Act as liaison for telehealth platform / software issues.
  • Analyze real-time data for virtual care eligibility and coordinate transitions.
  • Provide real-time technical support ("White Glove Service").
  • Continuous patient observation for safety, early-warning clinical monitoring, and reporting.
  • Document behavior / interventions; suggest discharge from remote monitoring when appropriate.
  • Monitor Epic telehealth orders / visits and ensure sessions run smoothly.
  • Follow HIPAA, escalate safety / equipment issues, maintain accurate records.
  • Submit shift reports; attend meetings; maintain attendance and a safe environment.
