Job Description :
Screens and evaluates patients / residents and develops appropriate care plan.Under a physician's direction, provides direct patient / resident treatment based on established plan of care.Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.Administers, scores, and interprets specialized hearing and speech tests.Instructs residents / patients, families, and caregivers to monitor speech and provide ways to practice new skills.Completes required forms and clinical records in accordance with company policy and state and / or federal regulations.Documents in patient records in an accurate and timely manner.Devises special tools and treatments for the specific needs of patients.Provides comprehensive and compassionate therapy services in a sub-acute / skilled nursing facility setting.Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment.Maintains resident / patient privacy pursuant to HIPAA.Ensures punctuality and regular attendance for assigned shifts.Knowledge / Skills / Abilities
Current knowledge of effective speech therapy techniques.Familiarity with clinical needs of geriatric population.Strong clinical capabilities.Requirements :
Active license in Speech Language Pathology in the Stat of Arizona.Bachelor of Science degree in Speech Language Pathology required.Certifications :
BLS (American Heart Association ONLY)Level 1 Fingerprint Clearance CardOIG / SAM name clearance checkTB Skin TestCOVID vaccination or declination of offered vaccineRole : Speech Language Pathologist
