Travel Nurse Case Manager – Poughkeepsie, NY

Assignment Length : 13 Weeks

Start Date : 11 / 10 / 2025

Shift : 8-Hour Days (8 : 00 AM – 4 : 30 PM, every 3rd weekend and every other holiday)

On-Call : None

HumanEdge Health is seeking a travel Nurse Case Manager for a 13-week assignment at a 349-bed acute care teaching hospital, Level 2 Trauma , in Poughkeepsie, NY . This role focuses on acute care case management, discharge planning, and care coordination for hospitalized patients.

Position Overview :

Conduct hospital transition-of-care and discharge planning

Coordinate with interdisciplinary team to optimize patient outcomes

Perform hospital case management including assessments, care planning, and documentation

Assist with internal floating within the department as needed

Ensure compliance with hospital policies and regulatory requirements

Requirements : Required :

NYS RN License

NYS PRI Certification

1–2 years of acute care discharge planning experience

Profiles must clearly demonstrate experience in discharge planning

Preferred :

Strong communication and organizational skills

Ability to work independently and as part of an interdisciplinary team
