Home Health Care Coordinator

A company is looking for a Home Health Referral Care Coordinator to support their referral coordination team.

Key Responsibilities

Coordinate home health and IV therapy referrals and assist with intake as needed

Ensure accurate processing of referrals by obtaining necessary documentation and promoting positive relations with referral sources

Manage inquiries and follow up with stakeholders while ensuring compliance with regulatory requirements

Required Qualifications

Some college education and / or medical background, with LPN preferred

Preferred experience in Work Comp Insurance, referrals, and authorizations

Proficient in medical terminology and moderate PC skills, including MS Office

Ability to work cohesively with coworkers and external customers with minimal supervision

Knowledgeable of injured workers' rights and able to maintain confidentiality of data
