Do you have a heart for helping others and a passion for supporting seniors? We’re looking for a Social Services Assistant to join our skilled nursing facility team! In this role, you’ll work side-by-side with our Social Services Director to ensure residents and their families feel supported, heard, and cared for—improving their quality of life one interaction at a time.

What You’ll Do :

Partner with the Social Services Director to create and carry out a meaningful Social Services program.

Support residents and their families by identifying social, emotional, and medical-related needs—and helping find the right solutions.

Provide counseling, guidance, and advocacy to those navigating personal or environmental challenges.

Collaborate with other department leaders to ensure residents’ needs are always at the center of care.

Keep accurate records that reflect the highest standards of compassionate care.

What We’re Looking For :

Bachelor’s degree in Social Work (preferred) or Human Services.

Previous social services experience required; long-term care experience strongly preferred.

State-required certifications / licensure (if applicable).

Someone who is empathetic, proactive, and truly loves making a difference.
