Field Nurse (RN) – Direct Hire Opportunity

The Field Nurse will provide comprehensive patient care in the home setting, ensuring quality outcomes and strong communication across interdisciplinary teams. This is an excellent opportunity for an RN who thrives in independent, patient-centered work and values a collaborative care model.

Salary Range : $105,000 – $110,000 / year

Location : Bronx, NY

Employment Type : Full-Time, Direct Hire

Key Responsibilities

• Conduct home visits and manage all aspects of patient care in accordance with physician orders and agency policy
• Complete all required documentation accurately and promptly in HomeCare HomeBase (HCHB)
• Communicate effectively with physicians (PCPs), interdisciplinary teams, and supervisors
• Prepare and update Plan of Care (485) and all interim orders
• Maintain and update the medication profile and coordinate care across disciplines
• Review and complete case communication notes, follow-up items, and any incomplete documentation
• Participate actively in case conferencing and care coordination meetings

Pay Details : $105,000.00 to $110,000.00 per year
