Job Responsibilities :
Conduct nutritional assessments and develop individualized nutrition care plans.Monitor patients' progress and adjust diet plans according to medical conditions and recovery status.Manage nutrition therapy for conditions such as malnutrition, dysphagia, kidney failure, respiratory failure, GI disorders, and metabolic imbalances.Provide education and counseling to patients and families on therapeutic diet orders and nutrition management.Collaborate with interdisciplinary teams including physical therapy, respiratory therapy, pharmacy, radiology, and social services to ensure coordinated care.Ensure accurate documentation in the Epic charting system.Support patients in LTACH and inpatient rehabilitation settings (10-bed LTACH / ICU and 46-bed inpatient rehab).Required / Preferred Information :
Experience : Minimum 1 year (Short Term Acute Care and Epic charting experience required)Certifications : BLS, RD / RDN, and valid Alaska state license.About HonorVet Technologies
