Position Overview
We are seeking an experienced Home Health Physical Therapist to join our Washington, DC team. The ideal candidate is passionate about improving quality of life for patients, confident in their clinical skills, and excited to collaborate with a supportive interdisciplinary care team. This is a great opportunity to work independently while still benefiting from team collaboration.
Responsibilities
Conduct comprehensive patient assessments of physical and functional abilities
Develop individualized treatment plans based on physician recommendations and patient goals
Deliver skilled therapy interventions to improve mobility, strength, balance, and daily function
Educate patients and caregivers on therapeutic exercises, safety, and prevention strategies
Monitor progress, adjust plans as needed, and ensure documentation is accurate and timely
Collaborate with nurses, physicians, and other professionals for coordinated care
Supervise Physical Therapist Assistants (PTAs)
Ensure compliance with HIPAA, Medicare, and all regulatory standards
Stay current through professional development and best practice updates
Qualifications
Graduate of an accredited physical therapy program with an active DC license
3+ years of clinical experience (home health or skilled nursing preferred)
Strong clinical assessment, planning, and intervention skills
Excellent interpersonal and communication skills with patients, families, and team members
Proficiency in EMR systems (Kinnser / WellSky or Homecare Homebase preferred)
OASIS experience strongly preferred
Reliable transportation and valid driver’s license required
