Job Description

American Traveler is seeking a Physical Therapist for a weekday Home Health assignment requiring at least one year of recent home health experience and current BLS certification.

Responsibilities

  • Home health setting with care provided in patients’ homes
  • Shift is Monday through Friday, 8 : 30am to 5 : 00pm with no weekends or on-call required
  • Assignments last 13 weeks
  • Patient assessment, discharge, evaluation, OASIS, and plan of care development required
  • Documentation in Netsmart (McKesson) EMR
  • Experience with adaptive and assistive devices, pain assessment, and patient / family education preferred
  • Mileage is reimbursed at the IRS standard rate
  • First-time travelers are accepted
  • Flu vaccine required unless medical exemption is documented; no religious exemptions accepted
  • Orientation is biweekly and requires completion of online modules prior to start
  • Previous perm employees cannot return as travelers for two years after last date worked
  • Current employees of the health system are not eligible for travel contracts at this facility
  • No radius rule applies for this position

    • Requirements

  • Current West Virginia PT license required (license required if applicable)
  • Minimum one year of recent home health PT experience
  • BLS certification required
  • Proficiency in OASIS documentation required
  • Strong time management and documentation skills
  • Ability to perform and document assessments, develop patient care plans, and handle long- and short-term goals
